How to Get Legendary Pacemaker Repkit in BL4
You can get the Pacemaker Repkit as a drop from Horace, who can be found in Hugering Plain in the Fadefields. It’s suggested that you unlock the Shut-Eye Keep Sake House, as it ensures you get there quickly. Defeat him at Horace’s Oversight, a location you might have visited during the Down and Outbound mission.
Before you start farming for the Mini Pacemaker Legendary Repkit, it’s important to note that you can only do so if you have completed the entire Down and Outbound mission.
Now, just use the Moxxi’s Big Encore machine to respawn him and farm him till he drops the Pacemaker Repkit.

Alternate Drop
Although the chances are slim, you can get the Pacemaker Repkit from Legendary Loot Sources like red chests, golden key chests, and other bosses. I don’t suggest this method, as farming Horace is more efficient and the odds are greater too (1 in 20). A consistent damage output weapon will end the fight quickly.
Pacemaker Repkit features burst healing abilities with added passive regeneration, adding up over time. Just keep an eye out for its 21-second cooldown time, and you are good to go.
