You will be met with several Companion side quests throughout The Outer Worlds 2, one of it being the “A Most Extraordinary Specimen” side quest. Like any other Companion quest, you will be met with two decisions. You can either kill the Grafting Doctor Giles Rasmussen or let him live and take him back to Ethel’s Lab. Not sure which option is the best for you? I’ll explain everything here.
Should I Kill or Spare the Grafting Doctor in The Outer Worlds 2?
Firstly, talk to the Grating Doctor to start the confrontation between him and Inez. The Doctor’s deeds are not that appreciable, as he was conducting grating experiments on creatures. Among those was Inez’s close friend, Trooper Ortiz. They died as their body rejected the grafting process.
If you decide to kill the Grafting Doctor

Here is the outcome if you choose the option to kill Grafting Doctor Giles Rasmussen:
- You will obtain one Augmentation Device and the Mechanical Augmentation.
- Inez will be satisfied with your decision.
During the conversation between Inez and the Doctor, Inez will pressure him on the ones who died just for the sake of his experiments. The Doctor will then try to bribe Inez by offering her state-of-the-art Grafting Parts for the sake of his life. Now, Inez will leave the decision to you.
Choose the “This is a personal choice, Inez. I’ll support whatever you decide.” dialogue option to kill the Grafting Doctor.
Inez will shoot the Doctor in the head. Head back to Ethel’s Lab at the Fairfields to end the “A Most Extraordinary Specimen” companion quest in The Outer Worlds 2.
If you decide to spare the Grafting Doctor

Pick the “You can set the terms, Inez. If things work out, so many lives can be saved.” dialogue option to spare the Doctor. To unlock this option, you must have Speech Level 17; otherwise, it will be locked. Finish the conversation and head back to Ethel’s Lab at Fairfields to install either of the Grafts. This will bring the “A Most Extraordinary Specimen” quest to an end.
This is definitely the best option to pick, as this will allow Inez to save many more lives and avenge Ortiz in the process. Besides that, you’ll obtain two Grafts for free, from which you can install one on Inez. Inez will also be satisfied with this decision, and the game will progress more smoothly.
However, know that if you spare him you will have to pick up the components for yourself Inez. Go back to the lab and get the components there.
Note: Sparing the Doctor is the best decision.
